District Nurse

District Nurses are specialist practitioners who can assess and, if appropriate, provide nursing care in your own home.

Patients who receive care from the district nursing service are fully housebound. Some of the areas that District Nurses can assist with are:

  • Assessment of patient needs including the needs of family and carers.
  • Anticipatory care planning for future needs
  • Wound management and nurse prescribing of wound care products
  • Post operative nursing support & care following discharge from hospital
  • Rehabilitation and health promotion
  • Monitoring of patients with long term conditions and chronic disease
  • Attend to patients with palliative/terminal care needs
  • Portable chemotherapy at home
  • Enteral and Parenteral feeding at home

Core Service

Monday – Sunday – 8.30 am – 4.30 pm

Out of hours service

Monday – Sunday – 4.30pm – 8.30am
